Thank you for supporting your child’s participation in the KAPSULE Ambassador Program.

Because the participant is under 18, we require consent from a parent or legal guardian before sending a complimentary KAPSULE product or beginning the collaboration.

Content & Image Permission

The separate Content & Image Permission field below asks you to accept or decline the following permission. Choosing No does not prevent participation in the program.

I give KAPSULE LLC permission to repost photographs, videos, and social media content voluntarily created and shared by the participant featuring KAPSULE products on KAPSULE-owned social media channels and the KAPSULE website.

This permission does not include paid advertising. Separate parent/guardian permission will be requested before using the participant’s image or content in paid advertising.

Shipping Information

This information will be used to arrange shipment of the complimentary KAPSULE product.

Privacy Notice
